A short term (30 days or less) group care facility that will assist youth and their parent or guardian through a screening process to identify needs, community resources, and a discharge plan in collaboration with the family and referring agency.

Services offered include individual therapy, treatment planning, mental health evaluation, group therapy, collaboration with parents/guardians/ foster parents, referral agency, schools or other provider, and educational services provided in program.

The Ryan White Part C program helps HIV positive people who live in Eastern SD to access primary health care. All patients who apply for the Part C Program will have access to case management services. The case managers help people with HIV/AIDS find services they need such as medical care, health insurance assistance, assistance with paying for medications, financial help, food/nutrition assistance, housing, referral to substance use treatment programs and/or mental health services, transportation, and other types of services. The Part C Program is a payer of last resort. Patients that are eligible for compensation through other programs, such as Medicare, Medicaid, or private health insurance, must first access assistance through those programs.
Funds are available to assist with primary care, lab testing, emergency prescriptions, outpatient mental health therapy, outpatient substance abuse treatment, dental care, transportation, outpatient nutritional services, and medical and non-medical case management.
Other services include nurse case management, social work case management services, and HIV testing.
