Provides a full range of treatment and rehabilitation services for adults with severe mental illness. The goal is to prevent hospitalization and to provide support and education; which, in turn, promote optimal levels of independence in living and working in the community. Services include psychiatric and medication management services, CARE (continuous assistance with recovery and empowerment), case management, supportive counseling, assisted living, vocational services, homeless outreach program, and IMPACT (individualized mobile program for assertive community treatment) services. A residential group care program for males, who have adjustment problems, between the ages of 9 and 18 years old and an independent living program for males between the ages of 17 and 20 years old. Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Provides 24 hours a day / 7 days a week overnight residential services to stabilize people experiencing an acute mental health crisis. Services include crisis stabilization, evaluations and and linking individuals to the services that will allow success in the long-term. Up to four individuals at at time may stay up to three days. There is no fee for services. Individuals in need of transportation to the facility may contact the facility or law enforcement to assist with transportation. DCI/Downtown is an Appropriate Regional Facility for the State of South Dakota offering safe, supportive services for individuals experiencing mental health crisis or thoughts of self-harm.
