Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Provides a full range of primary healthcare services including physical exams, chronic disease management, acute illness management, physicals, well child exams, children/adolescent/men/women healthcare services, immunizations, telemedicine related to medical and mental health needs, certified laboratory services, All Women Count services, assistance with Marketplace insurance enrollment, and allergy injections. Some sites provide Department of Transportation physicals. Nurse triage available for after-hours calls. Situation will be assessed by a registered nurse with further guidance provided. Strives to meet the needs of all patients, including the underinsured and underserved and offers services at a reduced rate to those who qualify.

Specialty care clinics available: cardiology, wound care, OBGYN Prescription Assistance - Enrolled members of a Native American tribe can get free prescriptions by visiting their local IHS.

Behavioral health services including addictive disorder services (ADS), compensated work therapy, Residential Post Traumatic Stress (PTSD) program, Mental health intensive case management, inpatient psychiatric services, and outpatient psychiatric services.
