Offers care of any acute medical problems and also offers STD testing and free flu shots. Psychiatry services also available during two appointment times (5:30pm and 6:30pm). The first clinic of each month will be for seeing new patients at these appointments while the second clinic will be for follow-up and seeing established patients. This clinic also offers prescription assistance.

Outpatient medical clinic, provides general family practice medicine. Prescription Assistance - Enrolled members of a Native American tribe can get free prescriptions by visiting their local IHS. Services include comprehensive primary care, Primary Care Medical Home (PCMH), behavioral health, dental, laboratory, nursing, pharmacy, public health nursing, radiology, dietitian, and telehealth (including behavioral health, internal medicine, and rheumatology).

Behavioral health services including addictive disorder services (ADS), compensated work therapy, Residential Post Traumatic Stress (PTSD) program, Mental health intensive case management, inpatient psychiatric services, and outpatient psychiatric services. Gun locks are available through the Suicide Prevention Coordinator.

Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Services include obstetrics with ultrasound, delivery with intrathecal block, pediatrics with inhalation therapy and IV therapy, family practice, geriatrics, disease management, hormone replacement therapy, and 24-hour emergency care and outpatient hospital.
