Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Loving School at Sioux Falls Children's Home and Owen School at Black Hills Children's Home serves students ages 4-14, in grades PreK through 8th grade, including children in the Sioux Falls and Black Hills Residential Treatment Program. Specializes in meeting educational needs relating to childhood trauma, attention deficit hyperactivity disorder, reactive attachment disorder, abuse and neglect, anxiety disorder, aggression, and parent-child challenges. Students receive individual and group therapy at a minimum of 2 hours per month and have access to medical care and psychiatric services, as needed. A student does not need an Individualized Education Plan (IEP) to be enrolled in Children's Home Society schools. If a student does have an IEP, the IEP is implemented and managed by Loving School staff or Owen School staff. Each student's treatment team meets quarterly to share progress and monitor treatment goals. Occupational Therapy and Speech/Language services are available as prescribed by an IEP.

Pharmacy services include immunizations, prescription refills, health screenings, and a medication disposal drop box. - Medication disposal drop-boxes for expired, unused or unwanted controlled, non-controlled, and over the counter medications, including pet medications. Items that are NOT accepted include: trash, regulated medical waste, sharps, syringes, thermometers, hazardous waste, aerosols, illicit drugs, or commercial waste.

Caseworkers work with women who are going through an unplanned pregnancy. Support services are available to clients at no-cost. Includes: options counseling, evidence-based decision making, adoption planning, adoption parent selection, parenting preparedness program & courses, before & after birth support, mental health counseling, Medicaid & WIC applications, transportation assistance, case management, job application & resume writing, help with difficult family or partner communication, community & state resource connection, and referrals related to housing, substance use, and domestic violence.

Strives to bring an authentic relationship in a comfortable, safe and supportive setting for all to be both seen and heard. Specializes in working with children ages 4-11 utilizing play therapy to meet and communicate with children in their world and in their primary language - play. Services offered include: individual counseling; child abuse counseling; couples counseling; play therapy; trauma informed therapy; faith-based counseling; cognitive behavioral therapy; adolescent/youth counseling; internet (telehealth) counseling; and EMDR (Eye Movement Desensitization and Reprocessing).

Hospice Care is special care for patients who have a diagnosis of a terminal illness or a significant decline in overall health. Hospice care can be set up at home, in long-term care facilities or assisted living, or hospice centers. Hospice professionals are specially trained to manage pain and symptoms with palliative medicine at the end of life so patients are comfortable, yet alert and able to enjoy each day to the fullest extent possible.

Outpatient mental health counseling center providing 100% free services to eligible Veterans, active duty service members, and their families. Part of the US Department of Veterans Affairs but separate from the VA Medical Centers, with separate and confidential medical records. Offering individual, group, and couples counseling both in Rapid City and at a number of Community Access Points in the Black Hills. Also, a referral service and can point Veterans in the right direction for any information they may need in terms of accessing benefits, enrolling in VA health care, etc. Specialty is helping Veterans readjust successfully into civilian life, and provides counseling for a wide range of problems, from mental health issues like PTSD, depression, and anxiety to life stressor issues such as work-related or relationship difficulties. Provides free bereavement counseling for loved ones of service members who died while on active duty or Veterans who died by suicide.

Provides pharmacy services, vaccinations, free local delivery, and is a medical disposal take-back site. Customers can use the RefillRx Mobile App to refill medications, set up medication reminders, and get text notifications for when prescriptions are being delivered.

The Care Center is the hub of many resources at the Center of Hope. Services provided include computers and phone/fax which guests can use, at no charge, for resume and employment/job searching. ​Pastoral care is available for individuals to receive one on one counseling with the pastoral team, including getting connected with the Earn-Your-Way program. Provides the following education and support services: Crisis Hot Line is a 24-hour service for anyone seeking help or information on domestic violence, stalking, or sexual assault. Domestic violence and sexual assault survivor advocacy services. Trained advocates are available to provide non-judgmental support for survivors, assistance with protection orders, referrals for medical needs/mental health/and legal services, food, personal items, diapers, other immediate needs, referrals for employment and education, and transportation to flee the area or for employment and housing search. Support and resource information by trained advocates to provide referrals for medical counseling, social services, and legal services depending on the victims needs. ​Confidential adult and children support groups which meet weekly. Available to all victims/survivors of past or present domestic violence, sexual assault, and stalking. Call for more information on available groups. Community education programs providing speakers, videos, and materials to the public to increase education and awareness on the issues of domestic violence, sexual assault, and stalking. A 27-week educational program for individuals who abuse their partners, Domestic Abuse Intervention Project (D.A.I.P.), is also available. Call for more information on community education programs.

Co-Dependents Anonymous is a fellowship of people whose common purpose is to develop healthy relationships. The only requirement for membership is a desire for healthy and loving relationships. CoDA uses the 12-Step and 12-Traditions as the principles and guides to develop honest and healthy relationships. In-person, Online and phone meetings are available to all. In-home case management for children, youth, and/or families that are at risk of system involvement (child welfare, juvenile justice, criminal justice, etc.). Assists caregivers strengthen their families and prevent child abuse and neglect through education, skill development and therapeutic crisis management.

Boys' Health Program is offered to those who may be at risk of developing physical, mental, or social health problems. Boys are referred to the program by school counselors, teachers, parents, health service providers, and social services agencies. Once enrolled, boys work closely with advocates to set and attain individualized goals. This program provides health-focused case management, education, advocacy, and prevention services. The Boys' Health Program helps lay the groundwork for lifestyle habits and attitudes that will strongly influence boys' health and behaviors as adults.

Specializing in food and weight issues; areas of expertise covers the full spectrum of disordered eating. Focuses on compulsive / emotional eating / binge eating disorder and weight loss coaching. Services provided include individual counseling / coaching, groups, and personalized small groups, classes, and workshops. Offering telehealth services. Prefer first session in person.
