Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, diabetes care, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Provide addictions assessments and mental health therapy to adolescents through adults. Community mental health center offering the following services: Moral Recognition Therapy (MRT) Group for adolescents and adults, Dialectical Behavioral Therapy (DBT), individual counseling, marriage and family counseling, psychotropic medication management, domestic violence evaluations and follow up services, psychiatric evaluations and medication management. Walk-in assessments are available at the Watertown and Sisseton locations. Also offers and telehealth services.

It provides Screenings / Assessments, Observation, De-escalation / Stabilization, and Referrals to appropriate services. Provides peer support and financial assistance to individuals and families who have lost a loved one to suicide. Examples consist of financial assistance for grief counseling, burial expenses, clean-up costs, legal fees, and other suicide-related expenses. The Program Coordinator supports clients with referrals and resources to additional financial assistance programs and/or therapeutic support if needed. Also provides peer and emotional support and resource referrals for a year. Peer support is provided through telehealth meetings and check-ins.

Services include Walk-in Urgent Visits (for both medical and dental), Schedule and same day appointments, Family Practice and Internal Medicine Primary Care for adults, Dental Care for all ages (including teeth cleanings), Sexual and Reproductive Health Services, On-site lab and X-Rays, On-Site Pharmacy, and After-Hours On-Call Provider 365 days a year. Primary and Pediatric Medical Clinic: Most routine medical services and several specialized fields including: physicals, immunizations, preventative medicine, chronic disease management, and counseling. Well child checkups, vaccinations, general pediatric health care. Mental Health: Offers outpatient counseling and psychiatric services, to both children and adults. Case management is offered to adults and families. Dental Clinic: Provides emergency, preventive, and oral health care. Accepts Medicaid, Private Insurance, Medicare, free of charge for the homeless and on a sliding fee scale for uninsured. One Heart Clinic: Providing primary family care to patients with a focus on patients with a primary language of Spanish. Pharmacy: Convenient, on-site location, CHCBH patients are able to fill most of their medications the same day as appointment.
