Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Detoxification Service: Couteau Des Prairies Hospital will provide emergency, acute medical detoxification services for those requiring these services. A Dakotah Pride Center staff member will provide transportation for detoxification services and a staff counselor will provide motivational counseling for clients who are undergoing detoxification to seek treatment through Dakotah Pride Treatment Center. Pre-treatment Evaluations: Provides screening, assessment, intervention, and treatment placement. Walk-in Assessments for Substance Use available. Adult Outpatient Program: Outpatient treatment services are provided for individuals who meet the admission criteria. The objectives of the program are to provide treatment for individuals who require therapeutic and supportive counseling and referral services while living independently, who may have completed inpatient treatment previously but relapsed, or for whom residential treatment is not otherwise possible. The program meets three times a week in three hour long sessions for six weeks. Adolescent Program: Provides outpatient treatment and continuing care services to adolescents that meet the eligibility criteria. Prenatal Empowerment Program (PEP): Works primarily with pregnant women who are struggling with substance addiction. A case manager will do addiction assessments, assist with medical applications and applications for Substance Abuse Treatment Centers, coordinate transportation for clients to off-reservation treatment centers, and conduct outpatient group sessions utilizing the Matrix Model to promote healing. Also, provides continuing care services and DUI offender classes.

Behavioral Health Services available from licensed substance abuse and mental health counselors. ​Services include: chemical dependency assessments and Level 1.0 counseling, individual, couples, and family behavioral health counseling, Medication Assisted Therapy (MAT) for patients recovering from substance use disorders, EMDR (Eye Movement Desensitization Reprocessing) Therapy, and psychiatry team availability. Transportation is available for current patients if needed.

Provides transitional, clinically-managed, low-intensity, open-ended residential treatment program for adult males. Treatment program is designed around an initial 90 day treatment plan. There are no guarantees as to the results of treatment. Services provided are a minimum of 12 hours of individual, group or family counseling per week. Residents are encouraged to take part in the outdoor operation of the facilities and participate in any of the recreational therapy programs. Intravenous (SP) (IV) Drug Users: Client referrals who are identified as having a history of using drugs intravenously (IV) will be prioritized for admission into this program.

There are three ways to schedule an appointment with LCBHS: Call to schedule an appointment Referral from your healthcare provider Referral following a treatment needs assessment from an accredited Substance Use Treatment Provider Individuals can often be seen on the same day that you call or are referred. Everyone will receive an assessment to determine whether or not MAT is appropriate/to help determine the most appropriate level of care. If MAT is determined to be an option, staff provide education about the different forms of MAT. Two forms of MAT are offered at LCBHS: Treatment with Buprenorphine Treatment with short- or long-acting Naltrexone A plan is developed with the patient's provider for the specific form of MAT treatment, level of care, and length of service based on your individual needs. Initial drug screening is required as are laboratory studies depending on history and type of MAT. Ongoing random drug testing is an integral part of treatment. Medically monitored withdrawal management is provided in a residential setting and is available if needed. There is not a set length of time for MAT. While many people may need to stay on medication indefinitely, others may choose to taper off or discontinue using MAT medications.

Provides individual, family, and couples therapy services including the following core issues: anxiety, depression, addictions, self esteem issues, couples issues, parenting challenges, and relationship building. Also offers leadership development services.
