Horizon Health - Isabel is full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Based on 12-step recovery model with medically monitored detoxification, Dual-diagnosis track with psychiatrist - For co-occurring disorders, diagnosis must be substance use primary and mental health secondary; Inpatient and outpatient gambling program with certified gambling counselors, Substance abuse, inhalant abuse and Methamphetamine treatment track (IMT), Chronic Pain Program, Medication Assisted Treatment - Different medications are available. Keystone utilizes Vivitrol and Naltrexone. May utilize Subutex and other medications to taper during medically monitored detoxification. Keystone does not utilize Methadone or Suboxone for maintenance. Unique services available are Native American cultural program (sweat lodge on site, talking circles, etc.), and a Spiritually based 12-step program. Veterans and retired military. CARE club provides support, assistance, interaction with others and the opportunity to improve consumers existing skills and develop new coping skills. Children's Services - Provides child centered therapeutic services in the home or office. This program provides mental health services to children with Serious Emotional Disturbance (SED). The parents, families, and surrogate families of children with SED participate in all aspects of evaluation, planning and delivery of services. Works closely with child-serving agencies and programs.
