Horizon Health - White River is full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. The Counseling Center offers the following services (all services based on eligibility and priority needs): Substance abuse services include screening, intake, evaluation, referrals, aftercare, primary prevention programs, education, MRT/SBISA classes, and group counseling. ​Mental health services include couples therapy, family therapy, individual therapy, group therapy, Education, Referrals, and Psychological evaluations and services.

Provides treatment for sleep disorders including insomnia and nightmares and other evidence-based interventions. Also provides mental health counseling to individuals to treat issues including depression, anxiety, life-adjustment stressors, grief and loss, trauma, substance use disorders, and behavioral addictions. Therapies include sleep therapy, Cognitive Behavioral Therapy for Insomnia (CBT-I), Image Rehearsal Therapy for Nightmares (IRT), Circadian Rhythm Sleep Wake Disorders, and CPAP Adherence.

Detoxification Service: Couteau Des Prairies Hospital will provide emergency, acute medical detoxification services for those requiring these services. A Dakotah Pride Center staff member will provide transportation for detoxification services and a staff counselor will provide motivational counseling for clients who are undergoing detoxification to seek treatment through Dakotah Pride Treatment Center. Pre-treatment Evaluations: Provides screening, assessment, intervention, and treatment placement. Walk-in Assessments for Substance Use available. Adult Outpatient Program: Outpatient treatment services are provided for individuals who meet the admission criteria. The objectives of the program are to provide treatment for individuals who require therapeutic and supportive counseling and referral services while living independently, who may have completed inpatient treatment previously but relapsed, or for whom residential treatment is not otherwise possible. The program meets three times a week in three hour long sessions for six weeks. Adolescent Program: Provides outpatient treatment and continuing care services to adolescents that meet the eligibility criteria. Prenatal Empowerment Program (PEP): Works primarily with pregnant women who are struggling with substance addiction. A case manager will do addiction assessments, assist with medical applications and applications for Substance Abuse Treatment Centers, coordinate transportation for clients to off-reservation treatment centers, and conduct outpatient group sessions utilizing the Matrix Model to promote healing. Also, provides continuing care services and DUI offender classes.

Services Offered: Individual psychotherapy for PTSD, trauma, depression, anxiety, loss-grief, addictions, communications, self-esteem, and behavioral issues and group therapy (Survivor Strong Groups for female survivors of sexual, physical and psychological abuse) through the following modalities: Eye movement desensitization and reprocessing (EMDR), motivational interviewing (MI), cognitive behavior therapy (CBT), rational emotive behavioral therapy (REBT), and dialectical behavior therapy (DBT).

Sober living homes utilizing: The Big Book of Alcoholics Anonymous and the way of life contained therein. Self-governing sober living homes that provide a safe living environment in early recovery with continued emphasis on applying the 12-steps of recovery to daily life over a period of 3 - 12 months. Advocacy of the disease concept by carrying the message to families, businesses, churches, and schools while building alliances with organizations that share the mission of advocacy for and implementation of the 12-step approach to recovery. Horizon Health - Isabel is also a partner with the Department of Veterans Affairs (VA) Community Care Program.

MAT (Medication Assisted Treatment) is offered to people who have moderate to severe opioid dependence. Because there is a complex relationship between health, mental health, and addiction, this treatment provides an integrated approach that focuses on the whole person. There are three ways to schedule an appointment with LCBHS: Call to schedule an appointment Referral from your healthcare provider Referral following a treatment needs assessment from an accredited Substance Use Treatment Provider Individuals can often be seen on the same day that you call or are referred. Everyone will receive an assessment to determine whether or not MAT is appropriate/to help determine the most appropriate level of care. If MAT is determined to be an option, staff provide education about the different forms of MAT. Two forms of MAT are offered at LCBHS: Treatment with Buprenorphine Treatment with short- or long-acting Naltrexone A plan is developed with the patient's provider for the specific form of MAT treatment, level of care, and length of service based on your individual needs. Initial drug screening is required as are laboratory studies depending on history and type of MAT. Ongoing random drug testing is an integral part of treatment. Medically monitored withdrawal management is provided in a residential setting and is available if needed. There is not a set length of time for MAT. While many people may need to stay on medication indefinitely, others may choose to taper off or discontinue using MAT medications.
