Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Community-based counseling center that provide a wide range of social and psychological services, including professional readjustment counseling to eligible Veterans, active duty service members, including National Guard and Reserve components, and their families. Bereavement services for family members is also available. Readjustment counseling is offered to make a successful transition from military to civilian life or after a traumatic event experienced in the military. In-Shelter or Out-of-Shelter services provided to victims and survivors of domestic violence and sexual assault, and their families, so they can make decisions about their lives in a supportive, non-judgmental and empowering atmosphere. Ongoing advocacy is provided and assistance with obtaining resources to seek, or rebuild, a new and safe life. Provides courtroom advocacy, assist with protection orders, can provide interpretive services, and safety planning, and can assist with supplies for day-to-day living and setting up a new household. Children are welcome. Assistance with transportation needs can be addressed. Shelter can accommodate nine adults and several children.

The Ryan White Part C program helps HIV positive people who live in Eastern SD to access primary health care. All patients who apply for the Part C Program will have access to case management services. The case managers help people with HIV/AIDS find services they need such as medical care, health insurance assistance, assistance with paying for medications, financial help, food/nutrition assistance, housing, referral to substance use treatment programs and/or mental health services, transportation, and other types of services. The Part C Program is a payer of last resort. Patients that are eligible for compensation through other programs, such as Medicare, Medicaid, or private health insurance, must first access assistance through those programs. Funds are available to assist with primary care, lab testing, emergency prescriptions, outpatient mental health therapy, outpatient substance abuse treatment, dental care, transportation, outpatient nutritional services, and medical and non-medical case management. Other services include nurse case management, social work case management services, and HIV testing.

Understanding Fetal Alcohol Spectrum Disorder: For children ages 5 - 11 and their birth, foster, or adoptive parents. While the parents or parenting team is involved in education, supportive discussion and training, the children will gather in a separate group to work on developmental and behavioral issues. - Mental Health First Aid Trainings: Offered periodically. Participants are taught to assess for risk of suicide or harm; listen non-judgmentally; give reassurance and information; encourage appropriate professional help; and encourage self-help and other support strategies. - Youth Mental Health First Aid Trainings: Offered periodically. Mental Health services include individual, couples and family counseling, 24-hour QMHP services, psychiatric evaluations and medication management, school consultations, and employee assistance programs (EAP). Alcohol and Drug Program treatment approach is based on the 12-step program of Alcoholics Anonymous and reality therapy.
