The IMPACT (Individualized and Mobile program of Assertive Community Treatment) program is designed for individuals with severe mental illness who are in need of intensive supports in the community. Services are highly mobile and delivered in the clinic, home or other community-based locations depending on need and consumer preference. Treatment is based on a comprehensive assessment and the patient is involved in each step of treatment planning. IMPACT services may include symptom monitoring, medication management, nursing care, individual or group counseling and education, direct assistance in obtaining basis necessities and other supports.
