Horizon Health - Aberdeen is full-service medical clinic offering the following services -preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Community-based counseling center that provide a wide range of social and psychological services, including professional readjustment counseling to eligible Veterans, active duty service members, including National Guard and Reserve components, and their families. Bereavement services for family members is also available. Intensive, non-residential treatment program providing services to children who meet the states' Severe Emotional Disturbance (SED) criteria and would not be successful in less restrictive services. A counselor along with family, individuals supporting the family, and professionals will develop a customized treatment plan which will assist in meeting the needs of the child and family. The parents, families, and surrogate families of children with SED participate in all aspects of evaluation, planning and delivery of services. Works closely with child-serving agencies and programs.
