Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

The Counseling Center offers the following services (all services based on eligibility and priority needs): Substance abuse services include screening, intake, evaluation, referrals, aftercare, primary prevention programs, education, MRT/SBISA classes, and group counseling. ​Mental health services include couples therapy, family therapy, individual therapy, group therapy, Education, Referrals, and Psychological evaluations and services.

​Individual, family, or group therapy for children, teens, and adults, who are victims or survivors or domestic violence, sexual assault, stalking, or child abuse. Services offered include: EMDR; child therapy for children ages 5 and older; crisis counseling for individuals staying in shelter; on-going therapeutic counseling with a masters-level therapist. Telehealth available.

The Adolescent Crisis Care Center provides immediate, short-term care for middle and high school youth who are experiencing a crisis or may be considering suicide. Services provided are mental health screenings/assessments, observation, de-escalation/stabilization, and referrals to appropriate services. Also has Community Health Workers that will provide case management services.

CARE club provides support, assistance, interaction with others and the opportunity to improve consumers existing skills and develop new coping skills. Children's Services - Provides child centered therapeutic services in the home or office. This program provides mental health services to children with Serious Emotional Disturbance (SED). The parents, families, and surrogate families of children with SED participate in all aspects of evaluation, planning and delivery of services. Works closely with child-serving agencies and programs.

Areas of expertise include anger management, addiction, anxiety, ADHD, Bipolar Disorder, chronic illness, Cognitive Behavioral Therapy (CBT), depression, grief, military/Veteran's Issues, OCD, PTSD, and Schizophrenia.

In-Shelter or Out-of-Shelter services provided to victims and survivors of domestic violence and sexual assault, and their families, so they can make decisions about their lives in a supportive, non-judgmental and empowering atmosphere. Ongoing advocacy is provided and assistance with obtaining resources to seek, or rebuild, a new and safe life. Provides courtroom advocacy, assist with protection orders, can provide interpretive services, and safety planning, and can assist with supplies for day-to-day living and setting up a new household. Children are welcome. Assistance with transportation needs can be addressed. Shelter can accommodate nine adults and several children.

Sessions often focus on exploring and shifting core beliefs through resolving past issues. For adults, sessions blend Cognitive Behavioral and Client Centered Therapies with Energy Psychology techniques to understand how their issues may be influenced by past trauma. By identifying dysfunctional core beliefs and resolving the events/emotions that helped create those false beliefs, clients can move forward with more joy, less stress and confidence in their own power to heal. Sessions utilize energy balancing and clearing techniques (Emotional Freedom Technique, Emotional Code/Body Code, Matrix Reimprinting and Vibroaccoustic Therapy) as appropriate to release blocked emotions and energy for clients of all ages. Flexible scheduling allows for sessions to be held in the office or other community settings when possible, and telehealth sessions are available.

Provides psychiatric residential treatment center licensed to serve boys and girls ages 10-17 years old, who have emotional, behavioral, and educational challenges. Under the supervision of credentialed doctoral level faculty, master's degree students provide quality therapy to families, couples and individuals in a safe and confidential environment. No one is turned away due to a lack of ability to pay. Adoption services offers domestic and international adoption services, including, but not limited to: support for individuals coping with the many emotions and decisions that come from choosing adoption; providing birth parents have the opportunity to interview and select the adoptive family; working under the provisions of Baby Moses law; and providing home studies and training for potential foster parents/homes. Pregnancy Counseling supports mothers as they experience the many emotions and decision that come with facing an unplanned pregnancy and help find the options that will work the best. Assistance with medical costs associated with pregnancy may be available. Some infant items are also available (layettes, infant clothing, etc.). Licensed social workers are available to travel and meet mothers involved in a crisis situation. Crisis pregnancy meeting sites will be kept confidential and be convenient for the mother. Start Making It Livable for Everyone (SMILE) classes for separating and divorcing parents with children under the age of 18, designed to: inform parents how to better understand the effects of divorce; assist parents to understand the needs of their children; help parents learn what they can do to create a nurturing and safe environment so their children can recover and feel good about themselves; inform parents how to prevent destructive game playing that is common among divorce; help parents gain problem solving skills; and let parents know they are not alone. Other programs include: Lakota Circles of Hope, Disaster Relief, and Uplifting Parents.
