Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Programs and services include a domestic violence shelter, a community crisis line with a toll free number, crisis intervention, and telephone or face-to-face crisis counseling and advocacy. Victim services include: criminal and support services, transportation, emergency financial assistance, referrals and assistance with housing/social services/legal services/other community resources, legal advocacy, and counseling referrals. Community education is available to schools, civic groups, or interested community members on topics of preventing domestic violence, dating violence, and sexual assault.

Outpatient program services for substance use and mental health (including co-occurring issues). Incorporates Lakota cultural traditional health along with mainstream modalities. Provides alcohol and substance treatment to the Rapid City (Mni Luzahan) Native American Community and other affiliated tribes in the region. The Native Healing Program utilizes the traditional Native American life ways as a method of healing the spirit. The Objective Zero App connects service members, veterans, their families, and caregivers to peer and civilian support through voice, video and text and to mental health and wellness tools and resources. The app is free to download and free to use.
