Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

The Mental Health Service Line provides connection to outpatient mental health services, psychiatry services, local recovery coordination, care coordination, and peer support services. Also offers care for military sexual trauma, PTSD, suicide prevention and awareness outreach, and gun safety locks. Telehealth service options are available.

Provides Buprenorphine Based MAT (Medication assisted treatment) to help people with an opioid use disorder abstain from or reduce their use of opioids. This center approaches patient care with a special emphasis on trauma-informed care as well as biological, psychological, and social concerns. - Inpatient Treatment: Available for patients ages 5 and older. All patients will be psychiatrically assessed and treated by a licensed and board-certified psychiatrist. Additionally, experienced nurses and patient care technicians provide individual specialized attention 24 hours a day, 7 days a week.
