Clinic provides primary care, mental health, home based primary care, and telehealth services. No emergency services are provided at the clinic. Specialty Services include: Audiology, Dermatology, Anticoagulation, and Home Based Primary Care Mental Health Services: individual, couples, and group psychotherapy Audiology Services: including hearing aid fittings, adjustments, and replacement, are provided on Tuesdays, Thursday afternoons and Friday afternoons at the CBOC. Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Provides traditional outpatient mental health therapy for individuals, families, children, couples, and groups. Also provides psychiatric evaluations, medication management, and psychological assessments. Services can be accessed both in person and via telehealth services. Children's services include case management and a home-based program that provides a wide range of therapeutic support. Early childhood mental health services are also available including play therapy. Areas of specialization include depression, anxiety, grief, parenting challenges, attention deficit hyperactivity disorder (ADHD), oppositional defiant disorder (ODD), stress management, and issues related to trauma or abuse.

Offers outpatient counseling services for individual, couple, and families. Staff members work with the client to provide services in the home, community, or office at the discretion of the client. Substance use evaluation, assessment, and treatment is also provided. Walk in assessments and telehealth services are available during normal business hours at the Aberdeen location. Psychiatric services are also available to those over the age of 18 including assessments and medication management.

Case Management and IMPACT Programs, designed to help clients with severe and persistent mental illness, services include case management and continued care, medication management with psychiatrist, crisis intervention assistance, and psychiatric rehabilitation which seeks to provide program services that assist clients in developing skills and manage resources needed to allow them to be successful in the community. A client centered, client directed form of treatment, the program consists of services designed to restore the disabled person to a level of functioning which permits a useful, meaningful life.

Cedar Village Assisted Living Facility is a therapeutic place designed to provide permanent supported housing for individuals who are homeless because of severe and persistent mental illness.

​ Sponsored by The University of South Dakota Sanford School of Medicine medical students under the supervision of internal medicine physicians, provide health care with the goal is to provide health care to uninsured patients over the age of 18 while providing a learning opportunity for medical students. Offers care of any acute medical problems and also offers STD testing and free flu shots. Psychiatry services also available during two appointment times (5:30pm and 6:30pm). The first clinic of each month will be for seeing new patients at these appointments while the second clinic will be for follow-up and seeing established patients. This clinic also offers prescription assistance.
