Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Provides medical, mental health, and complementary services in one location to ensure the best treatment outcome. Services are available to all people of all ages seeking healing from mental, emotional, and physical injury to improve health and wellness. Services provided include individual counseling for children, teens, young adults, and adults. Specialized treatments include art therapy, sex therapy, EMDR, and addictions counseling. Treats many conditions such as anxiety, depression, trauma, grief, abuse, domestic violence, attachment issues, behavioral issues, suicidality, and more. Also provides counseling for couples and families, and formal addiction assessments and treatment and group therapy for recovery. Psychiatry services, including assessment, diagnosis, and medication management, are available for individuals ages 3 and older. Provides functional medicine, acupuncture, massage, and yoga services, as well as supervision for LPC-MH and CSW-PIP licensure and consultation for EMDR certification.

Psychiatric care for people with anxiety, depression, mental illness, and addiction disorders. Staffed with a psychiatrist and counselor. Provides Buprenorphine Based MAT (Medication assisted treatment) to help people with an opioid use disorder abstain from or reduce their use of opioids. Can only serve people within a 50 mile radius of Aberdeen.

Approaching mental health treatment with a holistic approach. Utilizes a combination of medications, supplements, psychotherapy, and lifestyle modifications to improve mental health. Also provides an individualized treatment plan and offers referrals to therapists, counselors, dietitians, health coaches, and wellness professionals. Provides traditional outpatient mental health therapy for individuals, families, children, couples, and groups. Children's services include case management and a home-based program that provides a wide range of therapeutic support. Early childhood mental health services are also available including play therapy. Areas of specialization include depression, anxiety, grief, parenting challenges, attention deficit hyperactivity disorder (ADHD), oppositional defiant disorder (ODD), stress management, and issues related to trauma or abuse.

Clients receive help with depression, anxiety, behavioral problems, adjustment issues, grief and loss, as well as other mental health issues. An adult outpatient, time-limited, intensive, and coordinated clinical treatment program with the goal of stabilization with the intention of averting inpatient hospitalization or reducing the length of a hospital stay. ​Partial Hospital Program (PHP): Serves as a positive alternative when individual counseling does not fully meet the needs of the individual and when inpatient care is not necessary. 5 days per week, 4 hours per day. Intensive Outpatient Program (IOP): The goal of program is to provide support while promoting independence, growth, individuality and self-reliance. Clients are assisted in learning new and more effective coping skills while remaining in their homes and their jobs. Generally 7 - 11 hours of structured programming per week consisting of counseling and education groups.

A psychiatric hospital offering a wide range of services for geriatric, adult, adolescent, and child patients. This center approaches patient care with a special emphasis on trauma-informed care as well as biological, psychological, and social concerns. - Inpatient Treatment: Available for patients ages 5 and older. All patients will be psychiatrically assessed and treated by a licensed and board-certified psychiatrist. Additionally, experienced nurses and patient care technicians provide individual specialized attention 24 hours a day, 7 days a week. Inpatient Services: Specialized services include group and individual therapy, diagnostic testing, psychological evaluations, and family therapy. Recreational therapists provide a healing and therapeutic environment. Chemical Dependency Specialists perform detailed evaluations and treatment planning for patients struggling with addiction. Intensive case management is available to coordinate external services for the hospitalized patient. The hospital's clergy is available upon patient request or a patient's own spiritual leader is welcome to provide comfort. Hospitalization Process: If hospitalization is required, patients are provided a full physical examination and medical evaluation at the Rapid City Monument Hospital Emergency Department prior to hospitalization. Please proceed to the Emergency Department first or if emergent treatment is needed, call local law enforcement or 911. Outpatient Clinic: Offers individual and family counseling services for patients of all ages as well as psychiatric diagnostic interviewing and medication management for patients of all ages.

​ Sponsored by The University of South Dakota Sanford School of Medicine medical students under the supervision of internal medicine physicians, provide health care with the goal is to provide health care to uninsured patients over the age of 18 while providing a learning opportunity for medical students. Offers care of any acute medical problems and also offers STD testing and free flu shots. Psychiatry services also available during two appointment times (5:30pm and 6:30pm). The first clinic of each month will be for seeing new patients at these appointments while the second clinic will be for follow-up and seeing established patients. This clinic also offers prescription assistance.

The Mental Health Service Line provides connection to outpatient mental health services, psychiatry services, local recovery coordination, care coordination, and peer support services. Also offers care for military sexual trauma, PTSD, suicide prevention and awareness outreach, and gun safety locks. Telehealth service options are available.

Counseling services range from individual counseling, adult/adolescent psychiatry, cognitive behavioral therapy, and parent child interaction therapy.
