Alcohol and Drug Program treatment approach is based on the 12-step program of Alcoholics Anonymous and reality therapy. Services offered include individual counseling, chemical assessments, DUI education program, outpatient treatment, prevention education program, and a relapse prevention group. Case Management and IMPACT Programs, designed to help clients with severe and persistent mental illness, services include case management and continued care, medication management with psychiatrist, crisis intervention assistance, and psychiatric rehabilitation which seeks to provide program services that assist clients in developing skills and manage resources needed to allow them to be successful in the community. A client centered, client directed form of treatment, the program consists of services designed to restore the disabled person to a level of functioning which permits a useful, meaningful life.

Systems of Care Program | Dakota Counseling Institute

The Systems of Care (SOC) program is a wraparound approach to delivering services to at-risk youth and families. This program is a free resource available to children under the age of 21 and their families and is meant to be a temporary service to help meet the needs of the child and family. Identified needs include but are not limited to basic needs, social supports, emotional needs, educational needs, community supports, housing supports, health, and safety. This program is meant to be temporary where services will conclude once all needs have been met. In instances where the child or a family member requires additional supports outside of what can be provided through this program, assistance will be provided in obtaining these additional services.

Provides gas, electric or water utility assistance for bills at least 30 days past due with a shut off notice (cannot help with current bills). Call for qualifications and appointment time. TEFAP (The Emergency Food Assistance Program) is a federal program which helps supplement the diets of low-income families by providing emergency food and nutrition assistance at no cost. Option 1: TEFAP Food Only: Protein along with nutritional staple(s). Employment Assistance - Career planning, development of jobs search skills, interviewing, networking, and resume assistance. Exceptional Family Member Program (EFMP) - Provides information and referrals to local resources for Air Force Families with an exceptional family member (whether medical, emotional, physical or educational). Military and Family Life Counselor Program - Masters or doctorate-level licensed counselors provide short term, non-medical counseling services to service members and their families at no-cost (anger management, communication, relationship issues, conflict resolution, parenting, decision making skills, deployment stress, coping skills, homesickness, relocation adjustment, reintegration, separation, sadness, grief and loss). Personal Financial Management - Provides counseling and education with budget and savings management. This can be done in a one-to-one financial counseling setting or in a class. In conjunction with the American Center for Credit Education, provides classes such as Credit when Credit is Due, Money In Motion, and Home Buying classes for financial education. Readiness program - Assists military members and their families to prepare and go through deployments, TDY's, and remote tours. The Readiness program offers assistance with pre-deployment counseling, preparing practical aspects of separations, providing support to families, and offering reunion support.

HIV Positive Services | City of Sioux Falls Health Department

The Ryan White Part C program helps HIV positive people who live in Eastern SD to access primary health care. All patients who apply for the Part C Program will have access to case management services. The case managers help people with HIV/AIDS find services they need such as medical care, health insurance assistance, assistance with paying for medications, financial help, food/nutrition assistance, housing, referral to substance use treatment programs and/or mental health services, transportation, and other types of services. The Part C Program is a payer of last resort. Patients that are eligible for compensation through other programs, such as Medicare, Medicaid, or private health insurance, must first access assistance through those programs. Funds are available to assist with primary care, lab testing, emergency prescriptions, outpatient mental health therapy, outpatient substance abuse treatment, dental care, transportation, outpatient nutritional services, and medical and non-medical case management. Other services include nurse case management, social work case management services, and HIV testing.

Substance Use Care Coordination | South Dakota Resource Hotline

The South Dakota Resource Hotline is part of South Dakota's Statewide targeted response to substance use. The hotline provides a place where individuals are able to reach out for information on treatment resources and support for themselves or someone else. The Substance Use Care Coordination program: Connects participants with substance use resources, such as treatment assessments, treatment programs, counseling and social supports. Provides consistent connections during high-risk times for returned use, which may include post detox and before and after treatment. Provides support working on reaching recovery goals. Assists with making connections with appropriate community referrals for financial assistance, transportation assistance and assessing mental health resources. Connects with referral locations to best understand wait-lists, common barriers to treatment completion and share needed information between referral sources. For new referrals, fill out the Care Coordination Referral Form and the Care Coordinator will reach out in 1-2 business days.

Survivors Joining for Hope | Lost&Found

Provides peer support and financial assistance to individuals and families who have lost a loved one to suicide. Examples consist of financial assistance for grief counseling, burial expenses, clean-up costs, legal fees, and other suicide-related expenses. The Program Coordinator supports clients with referrals and resources to additional financial assistance programs and/or therapeutic support if needed. Also provides peer and emotional support and resource referrals for a year. Peer support is provided through telehealth meetings and check-ins.
