Also offers behavioral health services: medication management, counseling, case management, integrated care, addiction services, and Medication Assisted Therapy (MAT). Provides nursing facility and short-term rehabilitation care in a home-like environment with individualized care plans. Services include but are not limited to 24-hour nursing care, wound care management, nutrition services, safety systems, pain management, recreational activities, in-house pet therapy, physical/occupational/speech therapy, and restorative therapy. Also provides memory support, hospice, respite care services.

COVID Testing Option: ​Oyate Health Center Urgent Care 3200 Canyon Lake Drive 7:00am - 7:00pm (MT), 7 days a week Christmas Eve closes at 3pm (MT). Closed on Thanksgiving Day, and Christmas Day and New Year's Day. Oyate Health Center provides primary medical care, outpatient clinic, and urgent care. Additional outpatient services that are provided on site include optometry, pharmacy (Prescription Drug Take Back Site, blue drop-box by pharmacy), laboratory, radiology, dental, physical therapy, podiatry, and social services. Oyate Health Center provides direct health services to Native Americans and provides contract health care services and referrals to those that are eligible within IHS requirements. This means that if a patient is enrolled in a federally recognized tribe or can prove lineage of descent of an enrolled tribal member; the patient may receive direct care services at Oyate Health Center. Prescription Assistance - Any Native American enrolled in a federally recognized tribe can receive prescription expense assistance at any IHS facility.

The Ryan White Part C program helps HIV positive people who live in Eastern SD to access primary health care. All patients who apply for the Part C Program will have access to case management services. The case managers help people with HIV/AIDS find services they need such as medical care, health insurance assistance, assistance with paying for medications, financial help, food/nutrition assistance, housing, referral to substance use treatment programs and/or mental health services, transportation, and other types of services. The Part C Program is a payer of last resort. Patients that are eligible for compensation through other programs, such as Medicare, Medicaid, or private health insurance, must first access assistance through those programs. Funds are available to assist with primary care, lab testing, emergency prescriptions, outpatient mental health therapy, outpatient substance abuse treatment, dental care, transportation, outpatient nutritional services, and medical and non-medical case management. Other services include nurse case management, social work case management services, and HIV testing.
