Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, diabetes care, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Jerauld County Dental Clinic provides a full range of family oral health services including preventative care (check ups, fillings), restorative and cosmetic dentistry (crowns, bridges) , endodontics, and oral surgery (root canals, extractions).

The Ryan White Part C program helps HIV positive people who live in Eastern SD to access primary health care. All patients who apply for the Part C Program will have access to case management services. The case managers help people with HIV/AIDS find services they need such as medical care, health insurance assistance, assistance with paying for medications, financial help, food/nutrition assistance, housing, referral to substance use treatment programs and/or mental health services, transportation, and other types of services. The Part C Program is a payer of last resort. Patients that are eligible for compensation through other programs, such as Medicare, Medicaid, or private health insurance, must first access assistance through those programs. Funds are available to assist with primary care, lab testing, emergency prescriptions, outpatient mental health therapy, outpatient substance abuse treatment, dental care, transportation, outpatient nutritional services, and medical and non-medical case management. Other services include nurse case management, social work case management services, and HIV testing.

Services include Walk-in Urgent Visits (for both medical and dental), Schedule and same day appointments, Family Practice and Internal Medicine Primary Care for adults, Dental Care for all ages (including teeth cleanings), Sexual and Reproductive Health Services, On-site lab and X-Rays, On-Site Pharmacy, and After-Hours On-Call Provider 365 days a year. Primary and Pediatric Medical Clinic: Most routine medical services and several specialized fields including: physicals, immunizations, preventative medicine, chronic disease management, and counseling. Well child checkups, vaccinations, general pediatric health care. Mental Health: Offers outpatient counseling and psychiatric services, to both children and adults. Case management is offered to adults and families. Dental Clinic: Provides emergency, preventive, and oral health care. Accepts Medicaid, Private Insurance, Medicare, free of charge for the homeless and on a sliding fee scale for uninsured. One Heart Clinic: Providing primary family care to patients with a focus on patients with a primary language of Spanish. Pharmacy: Convenient, on-site location, CHCBH patients are able to fill most of their medications the same day as appointment.

COVID Testing Option: ​Oyate Health Center Urgent Care 3200 Canyon Lake Drive 7:00am - 7:00pm (MT), 7 days a week Closed on Independence Day, Thanksgiving Day, and Christmas Day. Oyate Health Center provides primary medical care, outpatient clinic, and urgent care. Additional outpatient services that are provided on site include optometry, pharmacy (Prescription Drug Take Back Site, blue drop-box in pharmacy), laboratory, radiology, dental, physical therapy, podiatry, and social services. Oyate Health Center has 2 sweat lodges that are by sign in only. Call for details. Oyate Health Center provides direct health services to Native Americans and also provide contract health care services and referrals to those that are eligible within IHS requirements. This means that if a patient is enrolled in a federally recognized tribe or can prove lineage of descent of an enrolled tribal member; the patient may receive direct care services at Oyate Health Center. Prescription Assistance - Any Native American enrolled in a federally recognized tribe can receive prescription expense assistance at any I.H.S. facility.
