Services include clinician on-call 24/7, pain management, guidance with Advanced Care Planning, forms, and financial care needs, provide medications, durable medical equipment, and supplies needed to treat life limiting illness, support and teach family/caregivers how to care for the patient, and grief/bereavement counseling for up to 13 months after the patient has passed away.
