Sanford Hospice is a specially trained team that is committed to providing our patients with quality end-of-life care. Our interdisciplinary team understands and is sensitive to the needs of our patients and their loved ones. Our team is comprised of nurses, medical director, social workers, dietitians, volunteers and chaplains. Hospice services can be provided wherever that patient calls home. This includes inpatient hospice, hospice cottages, nursing homes, assisted livings, and other long term care facilities. Hospice cottages, a unique way to care for a terminally-ill patient who is unable to remain in their home for care, are available, as well. ​​Ava's House features 16 adult patient rooms and 4 pediatric patient rooms. Home Health: Offered skilled home health services including nurses, physical therapy, occupational therapy, speech therapy, and home health aides providing care, support and education to allow patients to manage health conditions at home. Hospice: Offers a multidisciplinary team that addresses the patient and their family addressing physical, psychosocial, spiritual, and bereavement needs. Services include clinician on-call 24/7, pain management, guidance with Advanced Care Planning, forms, and financial care needs, provide medications, durable medical equipment, and supplies needed to treat life limiting illness, support and teach family/caregivers how to care for the patient, and grief/bereavement counseling for up to 13 months after the patient has passed away.
