Community Health Education | Sisseton-Wahpeton Oyate - Human Services Department

Offers patient education in the community and in schools to raise awareness on how to lower one's risk for chronic health problems and infectious illnesses. Topics include cancer awareness and screenings, injury prevention, tobacco prevention and cessation, sexually transmitted disease prevention, behavioral health awareness and screenings, drug and alcohol abuse awareness, immunizations, heart health, diabetes awareness and prevention, nutrition, and physical activity. Provides health education through health fairs, conferences, newspaper articles, health brochures, public service announcements, radio, posters, small group sessions, lunch n' learns, Facebook, and one-on-one counseling.

Delivers patient-centered, team-based care, and provides clinical services for: comprehensive ambulatory medical services for all genders and all ages, acute illness and chronic illness management, immunizations, children's well child exams, adult care, women's health, All Women Count, lab services, school/sport/commercial driver license (CDL) physicals, pre-operative exams, preventative cancer screenings, comprehensive diabetes program, health promotion and education, SD plan & family planning services, HIV & STD testing / STI prevention education, and medication assisted treatment, case management, and certified patient navigators. SDUIH is accredited by the Accreditation Association for Ambulatory Health Care (AAAHC) and is a Federally Qualified Health Center (FQHC). Transportation is available for current patients if needed.
