Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

An independent, nonprofit critical access hospital providing the following services: emergency care, cardiac rehabilitation services, coronary care, diabetes counseling, intensive care, in-patient services, obstetrics/childbirth services, occupational therapy, physical therapy, pulmonary care services, radiology services, respiratory therapy, sleep studies, swing bed program, surgical services, and on-site specialty providers (orthopedics, pain management, cardiology, urology, and audiology).

Urgent care offering services to the public such as allergy symptoms, bug bites, headache and migraines, common infections, foreign object removal, cold and flu symptoms, sexual transmitted infections (STI), lab testing, and DOT, drug screening and worker's comp.

Provides fully integrated medical and behavioral health care services and is a designated Primary Care Provider for Medicaid and Medicaid Health Home. Delivers patient-centered, team-based care, and provides clinical services for: comprehensive ambulatory medical services for all genders and all ages, acute illness and chronic illness management, immunizations, children's well child exams, adult care, women's health, All Women Count, lab services, school/sport/commercial driver license (CDL) physicals, pre-operative exams, preventative cancer screenings, comprehensive diabetes program, health promotion and education, SD plan & family planning services, HIV & STD testing / STI prevention education, and medication assisted treatment, case management, and certified patient navigators. SDUIH is accredited by the Accreditation Association for Ambulatory Health Care (AAAHC) and is a Federally Qualified Health Center (FQHC). Transportation is available for current patients if needed.
