The Part C Program is a payer of last resort. Patients that are eligible for compensation through other programs, such as Medicare, Medicaid, or private health insurance, must first access assistance through those programs. Funds are available to assist with primary care, lab testing, emergency prescriptions, outpatient mental health therapy, outpatient substance abuse treatment, dental care, transportation, outpatient nutritional services, and medical and non-medical case management. Other services include nurse case management, social work case management services, and HIV testing.
