Medical Services | South Dakota Urban Indian Health
Provides fully integrated medical and behavioral health care services and is a designated Primary Care Provider for Medicaid and Medicaid Health Home. Delivers patient-centered, team-based care, and provides clinical services for: comprehensive ambulatory medical services for all genders and all ages, acute illness and chronic illness management, immunizations, children's well child exams, adult care, women's health, All Women Count, lab services, school/sport/commercial driver license (CDL) physicals, pre-operative exams, preventative cancer screenings, comprehensive diabetes program, health promotion and education, SD plan & family planning services, HIV & STD testing / STI prevention education, and medication assisted treatment, case management, and certified patient navigators. SDUIH is accredited by the Accreditation Association for Ambulatory Health Care (AAAHC) and is a Federally Qualified Health Center (FQHC). Transportation is available for current patients if needed.

HIV Positive Services | City of Sioux Falls Health Department
The Ryan White Part C program helps HIV positive people who live in Eastern SD to access primary health care. All patients who apply for the Part C Program will have access to case management services. The case managers help people with HIV/AIDS find services they need such as medical care, health insurance assistance, assistance with paying for medications, financial help, food/nutrition assistance, housing, referral to substance use treatment programs and/or mental health services, transportation, and other types of services. The Part C Program is a payer of last resort. Patients that are eligible for compensation through other programs, such as Medicare, Medicaid, or private health insurance, must first access assistance through those programs.
Funds are available to assist with primary care, lab testing, emergency prescriptions, outpatient mental health therapy, outpatient substance abuse treatment, dental care, transportation, outpatient nutritional services, and medical and non-medical case management.
Other services include nurse case management, social work case management services, and HIV testing.
