Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Offers residential behavioral health support to young and old to help individuals cope and thrive in a nurturing environment. Services and amenities include 24/7 care under a registered nurse, private rooms, assistance with activities of daily living, medication management, activities to encourage mental and physical wellbeing, personal laundry and housekeeping services, nutrition services, supervised transportation, individualized plans of care, professional guidance for financial support, and payee services. Also has a partnership with Community Counseling Services.

The Union County Veterans Service Office (VSO) educates and assists veterans and their dependents and/or survivors in applying for veterans' benefits including benefits from the Department of Veterans Affairs and the State of South Dakota. Referrals are also made to other agencies that may have potential benefits for veterans. Areas of primary activity are obtaining military records and discharges, death/burial benefits, education benefits, home loan certificates, non-service connected pension, service connected compensation, state bonus program, survivors benefits, and veteran affairs health care. Veterans are encouraged to register their discharge information (DD Form 214, Report of Separation) with the Union County Registrar of Deeds Office.

Behavioral Health Services available from licensed substance abuse and mental health counselors. ​Services include: chemical dependency assessments and Level 1.0 counseling, individual, couples, and family behavioral health counseling, Medication Assisted Therapy (MAT) for patients recovering from substance use disorders, EMDR (Eye Movement Desensitization Reprocessing) Therapy, and psychiatry team availability. Transportation is available for current patients if needed.

Delivers patient-centered, team-based care, and provides clinical services for: comprehensive ambulatory medical services for all genders and all ages, acute illness and chronic illness management, immunizations, children's well child exams, adult care, women's health, All Women Count, lab services, school/sport/commercial driver license (CDL) physicals, pre-operative exams, preventative cancer screenings, comprehensive diabetes program, health promotion and education, SD plan & family planning services, HIV & STD testing / STI prevention education, and medication assisted treatment, case management, and certified patient navigators. SDUIH is accredited by the Accreditation Association for Ambulatory Health Care (AAAHC) and is a Federally Qualified Health Center (FQHC). Transportation is available for current patients if needed.

The County Veterans Service Officer (CVSO) assists veterans, dependents, and survivors by providing information, assistance, counseling, and referrals on a wide range of subjects, benefits, and veteran programs. CVSO's are trained to provide information on the following topics: Transportation assistance to VA Appointments, Offers complete claims service and explanation of VA procedures, Provides follow-up service once a decision has been made, GI loans, Compensation and pension, Survivors benefits, Education benefits, On-the-job and apprenticeship training, State benefits, Social Security benefits, Employment and unemployment, Small business development information, Corrections of military records, Review of discharges, Medical and dental treatment, Hospitalization or outpatient treatment, Nursing home care, Residency at the State Veterans' Home, Rehabilitation, Information and payment assistance for alcoholism and drug dependency treatment, Burial benefits, Obtaining flags, and Information on community programs. The CWOY Support Navigator program is designed to provide support for American Indian youth, adults, and their families in the He Sapa area who struggle with suicidal thoughts or behaviors. CWOY Support Navigators act as mentors, advocates, and resource guides for American Indian youth, adults, and families needing support for their healing and personal growth. Services offered include one-on-one mentorship, advocacy, individualized care planning, outings, activities and workshops, transportation to appointments, collaboration with behavioral health resources, coordination for success in academic programs, communication with family members, connecting participants to peer and community programs in support of healing goals, skill-building, mental health counseling, and therapeutic circles.

Detoxification Service: Couteau Des Prairies Hospital will provide emergency, acute medical detoxification services for those requiring these services. A Dakotah Pride Center staff member will provide transportation for detoxification services and a staff counselor will provide motivational counseling for clients who are undergoing detoxification to seek treatment through Dakotah Pride Treatment Center. Pre-treatment Evaluations: Provides screening, assessment, intervention, and treatment placement. Walk-in Assessments for Substance Use available. Adult Outpatient Program: Outpatient treatment services are provided for individuals who meet the admission criteria. The objectives of the program are to provide treatment for individuals who require therapeutic and supportive counseling and referral services while living independently, who may have completed inpatient treatment previously but relapsed, or for whom residential treatment is not otherwise possible. The program meets three times a week in three hour long sessions for six weeks. Adolescent Program: Provides outpatient treatment and continuing care services to adolescents that meet the eligibility criteria. Prenatal Empowerment Program (PEP): Works primarily with pregnant women who are struggling with substance addiction. A case manager will do addiction assessments, assist with medical applications and applications for Substance Abuse Treatment Centers, coordinate transportation for clients to off-reservation treatment centers, and conduct outpatient group sessions utilizing the Matrix Model to promote healing. Also, provides continuing care services and DUI offender classes.
